Child Dedication Request Form
Registration deadline for the May 13, 2012 Child Dedication is April 12, 2012. We can accomodate up to 10 families per dedication ceremony. Once we receive your registration you will be notified if you are confirmed for the May dedication.
April 19, 2012, 6:30pm –8:15pm: This date and time has been set as a must be at meeting with both parents prior to the Child Dedication. We will meet in the Main Conference Room on the 3rd Floor. At this time, you will be introduced to the Family Pastor, Children’s Ministry Director and Parent Liasons for Child Dedications. You will also be introduced to other families who will be dedicating in May. Dinner for you and your children (2 yrs and up) will be provided. Childcare will be provided by reservation only to Margaret Cary at .
In advance of April 19, 2012, please prepare a 1-2 minute verbal presentation including the following information: a brief introduction of you and your family and the child you are dedicating; the dedication verse you have chosen for your child, and something special that you might want to share about the child you are dedicating.
